Certain medical conditions (such as asthma, epilepsy, diabetes, etc) may prevent you from going diving. To ensure both your and our teams safety, RSTC require all divers to complete this form.
If you answer yes to any boxes marked with * please print this form as well as the professional guidelines (VHMS website) and take it to your physician for a medical evaluation. If approved please present a copy of this completed form on arrival.

Customer safety is our no.1 priority at Triton Explorers however even with standards and safeguards in place accidents can happen.
To make sure you fully understand the risks of scuba diving and the specific rights you are giving up in return for participation in our programmes we require all divers to sign a liability release like this one before engaging in any activities.
It is important that you take your time to read this document carefully and only sign if you understand and agree with everything it states.
We accept divers from all agencies for fun dives and courses. However every diver (unless participating in entry level programmes) must show proof of certification prior to departure.
If you do not have this on hand you may be able to search for past training on the certifying agencies website such as SDI or PADI. If you cannot find proof of your certification please get in touch to see if we can help.
For safety and Insurance purposes divers will not be permitted to dive beyond their training limits.
Accidents happen, especially on remote tropical islands – be it delayed/cancelled flights, poor weather or medical emergency – and if/when they do it can quickly put people out of pocket.
Most travel insurance can help cover these costs however many do not include (or have heavy limitations on) ‘extreme sports’ such as scuba diving.
Which means in the event of a dive incident, fees for evacuation and treatment in the nearest hyperbaric chamber on Port Villa will be at your own expense.
For this reason we strongly advise all our customers check and/or purchase travel and dive insurance prior to booking.
We have used Divers Alert Network (DAN) for many years and can highly recommend their services. With plans covering everything from recreational, technical and professional diving for temporary trips or year round protection.
A 24/7 emergency hotline and direct links to the hyperbaric chamber in Port Villa also make them a top choice when it comes to speed of delivery. This is true not only in Vanuatu but worldwide.
And since profits from DAN are reinvested in research, medical services and programs you know that the person on the end of the line will know exactly how to help. Even if hopefully you never need it!
